1. Comprehensive medical result (Chest x-ray, full blood count, genotype, urine analysis, stool microsocopy, pcv, HbsAg, Hcv, Rvs, MP, Widal, Weight(kg), Height(m), visual acuity(aided and unaided) and general physical examination
2. Letter of recommendation from church/mosque
3. Letter of recommendation from school head
4. Baptismal Card
5. Birth certificate
